Off track engaging Hill sachs ( rare to see on MRI)

Anterior dislocation of the left gleno-humeral joint with engagement.

A focal impaction fracture at anteromedial cortex of the humeral head with surrounding marrow contusions – *suggestive of Hill-Sachs lesion (engaging /off track Hill-Sachs lesion)*.

Anterior chondro-labral separation at 3-6 o’clock position associated with truncation of anterior inferior glenoid rim (3 o clock is anterior).

Anterior inferior labral glenoid bone loss, less than 15% (soft tissue as well as bony Bankart lesion)

Focal thickening of supraspinatus tendon at the level of crescent segment demonstrating fraying of the bursal fibers. No evidence of discontinuity or retraction.

Findings suggestive of anterior band HAGL.

Moderate amount of gleno-humeral joint effusion
