INTERNAL MESENTERIC HERNIA

Clockwise twisting of distal ileal loops in the right lower quadrant, along with mesenteric vessels anteromedial to the common iliac vessels (whirlpool sign) causing resultant upstream dilatation of the small bowel loops with multiple air fluid levels – represents Moderate grade distal ileal obstruction due to internal mesenteric hernia (a congenital defect in mesentery)

Small free fluid in the pelvis. No evidence of pneumoperitoneum.

The anatomic relationship between SMA – SMV is maintained. No evidence of congenital malrotation
