tuberculosis versus Crohn’s disease

Features suggestive of  partial distal small bowel obstruction due to short segment circumferential mural thickening of the terminal ileum  demonstrating  marked focal thickening of surrounding mesenteric fat (creeping fat sign) as described above.

 Mildly thickened caecum and proximal ascending colon with suspicious subcaecal fistula

 Hyperenhancing mucosal margins of the distal ileum with segment mural thickening of a proximal ileal loop  in the left quadrant suggesting skip lesion .

 A few marginally enlarged mesenteric lymph nodes  in right lower quadrant

 Tiny pneumonic infiltrates as described above.

 Features likely suggestive of tuberculosis versus Crohn’s disease.

 Recommend clinical correlation, HPE, chest imaging,  follow up.
