Autoimmune versus HSV encephalitis

Clinical History :8-year male presenting with convulsions with history of altered sensorium since last 15 days

Subtle patchy  illdefined T2 / FLAIR hyperintensities involving the left inferior temporal subcortical white matter and adjoining left posterior frontal subcortical white matter with slightly hyperintense left insular cortex without restriction of diffusion –likely related to  resolving Autoimmune versus HSV encephalitis 

 Differential considerations also include:- myotonic dystrophy

 No evidence of mesial temporal sclerosis in this study. No evidence of significant mass effect or mid line shift.
