Bilateral internal carotid artery stenosis/hypoplasia with compensatory ectasia/dilatation of bilateral vertebral arteries, basilar artery and resultant flow related large terminal basilar artery aneurysm.

An approximately 1.5 x 1.3 cm fusiform aneurysm  arising from the tip of basilar artery demonstrating normal flow voids

Age related volume loss with  minimal periventricular white matter chronic small vessel ischemia.

 No evidence of acute infarct or restricted diffusion on DWI.
