FLAIR-variable unilateral enhancement of the leptomeninges (FUEL)

Clinical history: 25 year female with headache , seizures and fever since 1 month

Focal effacement of the left ambient cistern with marked focal abnormal leptomeningeal enhancement in the left medial temporal region associated with focal thickening of left optic tract –of concern for a variant of myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD), such as FLAIR-variable unilateral enhancement of the leptomeninges (FUEL)

Differential considerations also include:- focal infectious / auto immune encephalitis versus neoplastic etiology.
