Interhemispheric epidermoid with dystrophic calcification.

*Small acute embolic infarct in pons (to right of midline)*

• An approximately 3.5 x 2.8 x 2.7 cm altered signal intensity interhemispheric mass lesion at the frontal region, impinging on the body of corpus callosum from superior aspect displacing the corpus callosum inferiorly, protruding into the bilateral frontal parafalcine regions (more marked on the left) – likely suggestive of interhemispheric epidermoid with dystrophic calcification.

• Lacunar infarcts in bilateral capsuloganglionic regions, corona radiata , pons

• Age related mild volume loss with Fazekas grade II periventricular white matter chronic small vessel ischemia.

• No evidence of significant mass effect or mid line shift.
