Diffuse FLAIR hyperintensity involving the entire pons, part of midbrain and part of middle cerebellar peduncles without restriction of diffusion on DWI – likely related to posterior reversible encephalopathy syndrome (PRES).
Differential considerations also include:- demyelination versus subacute brainstem infarct
Age related volume loss with Fazekas grade II periventricular, subcortical white matter chronic small vessel ischemia.
No evidence of acute infarct or restricted diffusion on DWI.
