T2-FLAIR MISMATCH SIGN – an imaging marked for IDH-mutant 1p/19q non-codeleted gliomas (diffuse astrocytomas). An approximately 3 x 5.8 x 3.3 cm ovoid poorly enhancing altered signal intensity lesion involving the right inferomedial temporal lobe, involving part of amygdala, hippocampus and the uncus with minimal perilesional vasogenic edema – represents low grade diffuse astrocytoma (WHO grade I to II)The lesion demonstrates T2-FLAIR mismatch at the central portion – which may suggest an IDH-mutant 1p/19q non-codeleted diffuse astrocytomasNo evidence of significant mass effect or mid line shift.
