unilateral thalamic venous infarct with dural av fistula

A focal T2/FLAIR hyperintensity involving the right thalamus ( hyperintense on T2,  FLAIR and hypointense on T1Wt sequence) causing mild right thalamic expansion  – likely suggestive of  hemorrhagic venous infarct

 A tiny filling defect at the anterior part of straight sinus with nonvisualization/non-opacification of the right internal cerebral vein – likely related to chronic venous thrombosis.

 Bunch of multiple tortuous enhancing abnormal vessels predominantly localized in the left posterior temporal, occipital regions which are apparently draining into the straight sinus, left transverse and sigmoid venous sinuses as well as the left internal cerebral vein – suggestive of dural AV fistula formation.

 Age related volume loss with Fazekas grade 0 to I periventricular, subcortical white matter chronic small vessel ischemia.
