The tibial tunnel is entirely anterior to and parallel to the line drawn along the roof of the intercondylar notch (Blumensaat line) with deformed graft near tibial attachment – suggestive of graft tear

Slight anterior subluxation of the knee joint.

• Small patchy marrow contusions involving posterior part of lateral tibial plateau.

• Small osteochondral impaction contusion / fracture at lateral femoral condyle (suggesting pivot shift mechanism injury)

 Status post ACL repair surgery with graft in place.The tibial tunnel is entirely anterior to and parallel to the line drawn along the roof of the intercondylar notch (Blumensaat line) with deformed graft near tibial attachment – suggestive of graft tear.

 Localized anterior arthrofibrosis in this region.

 Grade III tear of medial collateral ligament at tibial attachment.

 Grade III horizontal medial meniscal tear at posterior horn.

 Focal abnormal high STIR signal intensity involving the menisco-tibial ligament suggestive of *Ramp lesion type 3

 Grade III longitudinal , displaced complex lateral meniscal tear at posterior horn at the point of Wrisberg attachment (*Wrisberg rip).

• Moderate joint effusion with large prepatellar bursal effusion
