5.5 x 3 x 2.2 x 1.8 cm expansile mixed T2 signal intensity intramedullary cystic lesion involving the proximal metaphysis of right humerus with surrounding marrow edema, demonstrating thin septations/loculations– suggestive of simple bone cyst.

Tiny areas of cortical dehiscence with periosteal reaction and fallen fragment sign and fluid-fluid levels – suggestive of pathological fracture

Differential considerations also include:- aneurysmal bone cyst versus fibrous dysplasia.

No evidence of extraosseous soft tissue.
