diffuse astrocytoma’ (IDH mutatnt).

An extensive irregularly marginated altered signal intensity mass lesion measuring about 7×4.5cm involving right insular cortex, lentiform nucleus, adjoining capsuloganglionic region and right thalamus also infiltrating adjoining inferior frontal and temporal gyrus around the opercular region with focal perilesional vasogenic edema (evaluation is limited due to lack of IV contrast study) – represents neoplasm , likely ‘diffuse astrocytoma’ (IDH mutatnt).

The central portion of the lesion is partially attenuated on FLAIR suggesting T2 – FLAIR mismatch – suggesting radio genomics ‘IDH-mutant, 1p19q non-codeleted.’

No evidence of tumour bleed.

0.5 cm midline shift to the left due to mass effect.

Recommend IV Contrast study for complete evaluation, MRS, follow up
