The region of white matter is diffusely hyperintense on T2 extending from periventricular region to sub cortical u fibers and demonstrates diffuse fluid attenuation on FLAIR (suggesting early cavitatory changes) sequences Associated with abnormal T2 hyperintensities involving bilateral internal capsules, corticospinal tracts, cerebral peduncles, pons, middle cerebellar peduncles associated with restricted diffusions on DWI as described above – represents a genetic leukoencephalopathy, likely Vanishing white matter disease (VWM).

No evidence of significant mass effect or midline shift in this study. No evidence of hydrocephalus.
